Asuris voluntary dental products (for MBA/BIAW groups only)
Our voluntary dental products offer dental coverage that encourages consistent preventive care and covers most major services. Members and their families can receive preventive, restorative and major dental services from any provider they choose.
The two dental plans are available, Incentive Dental and Percentage-Based Dental, and include the following features:
- Dedicated Customer Service phone number: 1 (888) 849-4743 for benefits, eligibility and claims payment information.
- Member cards include a unique member number suffix: -XX. View a sample card.
- A specific claims submission address on the back of the member card. Before submitting any claims, please ensure they are sent to the correct location.

Incentive Dental Plan
Plan highlights:
- No waiting periods.
- The $50 per member deductible is waived for cleanings and examinations.
- Each year the member has an examination and cleaning, the annual benefit maximum and reimbursement levels will increase (up to four benefit levels) (see example below).
Percentage-Based Dental Plan
Plan highlights:
- Enrolled members can access care immediately and pay nothing out of pocket for preventive services
- Waiting periods apply:
- Six months for restorative services
- 12 months for major services
- Specific percentages for restorative and major services after the annual deductible is met (see table below).

Provider networks
Members can seek services from our Participating (PAR) or Preferred Provider Plan (PPP) dental providers or non-contracted providers.
