Cosmetic Codes
CPT and HCPCS codes that are considered by Asuris
to be cosmetic and ineligible for benefits are listed
below. Other codes on the Clinical Edit List that
are identified as potentially cosmetic will require
submission of documentation to determine eligibility
for benefits.
Asuris considers the following codes to be cosmetic
for commercial products (excluding Medicare).
