Our Vision Plus Eyecare Program gives you and your qualified
dependents great discounts on eyecare services and products
at over 50 participating providers nationwide. These
discounts apply to eye examinations and eyeglasses. The program also provides discounts
on LASIK eye surgery with selected providers. And, best
of all, you can use the program as often as necessary.
The Vision Plus program is not an added cost to your
benefits. It’s simply a convenient discount program
we’re making available to our members through
Cole Managed Vision.

To use the Vision Plus Eyecare Program simply
follow these steps:
- First, find a participating Vision
Plus provider.
- Next, identify yourself as an Asuris Northwest Health
Vision Plus Eyecare Program member when making an
appointment.
- At the time of service, give the provider your Vision
Plan number, 47135, to verify participation.
- The provider will then apply discounts at the time
of service.

Many people are talking about laser vision correction and specifically LASIK, a procedure that has enabled millions to significantly reduce or totally eliminate their need for eyeglasses or contact lenses. LASIK is an outpatient procedure that corrects nearsightedness, farsightedness and astigmatism. Ophthalmologists use a cool beam laser to reshape the cornea. The procedure is virtually painless with near immediate results for many individuals.

- One All-Inclusive Price – You will not be charged for pre- or post-operative care or enhancements during the first year following surgery.

If you like, you can arrange for a free evaluation to find out if you are a good candidate for the procedure. You can also obtain information about financing options that may be available to you.
What does it mean to be a “good candidate”?
While LASIK can be used to treat a wide range of refractive errors, this surgery is not for everyone. Our doctors carefully evaluate prospective patients, and turn away those who do not fit the candidate profile. This ensures that our network surgeons are performing LASIK on only those patients who can realize the benefits of LASIK*.
That is why we recommend that our members take advantage of the free evaluation provided by the doctors in our network. The evaluation determines if a participant is a good candidate by determining refractive error, corneal thickness, pupil size under low light conditions, and the overall health of your eyes. Approximately 20% of the patients who take advantage of the free evaluation present one or more reasons to not proceed with LASIK. That is why your vision program still includes discounts for exams, eyeglasses and contact lenses.
Financing options
As exciting as the savings of your LASIK program may be, LASIK surgery still does not always fit neatly into everyone’s household budget. That is why we have arranged two special financing options as part of your program. In addition to paying for LASIK with a check or credit card, qualified members may choose to take advantage of a low monthly payment plan or a 6-month, same-as-cash program. * LASIK is an elective surgical procedure. Determination of a member’s suitability for any medical procedure or treatment will be made by the surgeon. Any decision to proceed with this procedure will be the sole decision of the member following relevant examinations and consultations. Neither Asuris Northwest Health nor Cole Managed Vision will be responsible for any such determination or decision or outcome.

How do I enroll?
As an Asuris Northwest Health member, you are already enrolled. Because you are part of the Vision Plus Eyecare Program, you and your dependents can begin using this program now and as often as necessary throughout the year.
How do I use Vision Plus?
Simply call any of the participating providers to schedule an appointment. Identify yourself as an Asuris Northwest Health Vision Plus Eyecare Program member when making an appointment. Give the provider your Vision Plus plan number, 47135, to verify participation at time of service. The provider will then apply discounts.
Where can I go to use Vision Plus?
You can find the nearest provider on the provider list. Be sure to give the provider your Vision Plus plan number, 47135.
Do all provider locations have an Optometrist on site?
Most locations have participating licensed independent Doctors of Optometry on site. You may call your desired provider location to confirm whether a doctor is available to administer exams.
Where can I find a participating LASIK surgeon?

Can I get an eye examination from one provider and my eyeglasses or contact lenses from another?
Yes, unless you are a first-time contact lens wearer. In this case, you must purchase your new contacts from your exam provider and return for one or two follow-up visits to ensure your lenses are fitted properly. Doctors are required to release your prescription for eyeglasses to you.
Can I use my discount in conjunction with another plan or store promotion?
When you visit a participating provider that is running an in-store promotion, you can choose to use your Vision Plus Eyecare Program or the in-store promotion, whichever is preferable to you. You cannot combine the discount program with an in-store promotion or your Asuris Northwest Health vision benefit (if you subscribe to an plan with a vision benefit).
