Traditional Dental Plans
The Traditional Dental Plans include preventive and diagnostic services as well as basic, restorative and major services. We also offer various deductible and coinsurance level options to suit a group's needs. These plans are sold as a rider to our medical plans.

Columbia Dental Plans
The Columbia Dental Plans are prepaid managed care dental plans. There are no deductibles, annual plan maximums, and/or claim forms.

A per-visit copay is required for routine dental work, then all minor dental work and preventive services are covered in full. If major dental work such as root canal surgery, crowns, bridges, dentures, periodontal surgery or other services are necessary, an additional copay is required. All care must be obtained from a Willamette Dental Group (formerly Columbia Dental Group) office location.

The benefit information contained in the documents above is effective on the date stated on the bottom of each document. For renewing groups, benefit changes are effective upon the group's renewal unless otherwise stated.
