Jump Start Sales Program for EmbarkSM and VantageSM
How do you feel about selling Embark and Vantage to your small-group clients and earning a little something extra as well? You can feel confident every time you bring a new client—or renew an existing client —With Asuris. That's because you're offering more than a quality health plan. You're also offering your clients and their employees the tools they need to make their own health a priority—and that means a healthier community as a whole. That's a big deal, and one we want to recognize.
We'll pay qualifying agents for each Embark or Vantage group sold or renewed with effective dates of Nov. 1 2007, through Dec. 1, 2008. You can quality for this program by selling or renewing a minimum of 25 Embark or Vantage enrolled employees during the program period.

Program Rules
- This program is based on a per-group, per-enrolled-employee basis.
- Payment is issued once for each qualifying group. It will be issued after the qualification threshold is met and is retroactive to the first group qualified during the program period.
- An enrolled employee is determined based on enrollment data recorded in our system on the last business day of the group's effective month.
- Payment will not be adjusted for enrollment received after the end of the month from the group's effective month.
- Payment will be reported on Schedule A (Form 5500) to applicable groups.
- The term "broker" or "agent" refers to a company employing one or more producer who is appointed to sell Asuris products.
- We may, at any time, amend these rules.
