January 2012 GMA forms now available on our website
Last month, we announced the availability of our updated Group Master Applications (GMA) and GMA Guide to Changes in Producer Update.
Now, our fillable and prefilled GMAs (1 to 99 and 100+) for effective dates of Jan. 1, 2012, and later have been updated and posted to the Forms section of our Agent and Employer websites and the Agent Toolkits. Prefilled GMAs are in Agent Center and are available only for renewing groups.
As a reminder, ASC (self-funded) groups require a GMA. Those GMAs are available through your Asuris Sales contact.
The July 1, 2011, GMAs (1 to 99 and 100+) will remain posted on our Forms section of the Agent and Employer websites until approximately Jan. 15, 2012. Those GMAs are for new and renewing groups effective July 1, 2011, to Dec. 31, 2011.
