June deadline for new group submission dates approaching
All required forms must be complete and submitted to us by the published deadlines to guarantee effective dates and rates; no exceptions. Incomplete or inaccurate information will delay the effective date of group coverage.
The following information and forms are required:
- Group Master Application
- Application for Enrollment/Change or Enrollment Spreadsheet
- Waiver Form
- Affidavit of Qualifying Domestic Partnership (if applicable)
- Affidavit of Qualifying Incapacitated Dependent Eligibility (if applicable)
For groups with July 1, 2011, effective dates, enrollment information will be due on the following dates:
- June 20, 2011: Paper Enrollment Applications are due by 5 p.m. (Pacific time).
- June 27, 2011: Online or Spreadsheet Enrollment Information is due by 5 p.m. (Pacific time).
To be consistent with these deadlines, Agent Center will close for quoting at 5 p.m. (Pacific time) on June 20, 2011, for quotes for coverage effective July 1, 2011.

In March, we described New Subscriber Enrollment via spreadsheet, which is available to all new and existing (20+ employee) groups. Groups using the spreadsheet don't need to provide paper applications. Enrolling via spreadsheet makes it easier to provide enrollment data and reduces opportunities for data entry errors that can slow enrollment time. The Asuris Spreadsheet Enrollment template can be found on the forms page of our public website.
