New Asuris Individual EmergeSM applications and Standard Health Questionnaires
The application for our Individual Emerge plan has been revised. Also, as we mentioned in a previous article, the Standard Health Questionnaire (SHQ) exceptions page (page 2) for Washington State has been updated to reflect the new Under 19 Open Enrollment and Special Election Period rules.
The new application and SHQ are effective for coverage beginning on or after July 22, 2011. Applications or SHQ's with revision dates prior to July 22, 2011, won't be accepted after Aug. 31, 2011. You can determine if you're using the updated application and SHQ by checking the document's revision dates.
