Reminder: January deadline for new group submissions approaching
In an effort to deliver a better experience for our members, we announced in 2010 new group submission deadlines for groups of all sizes.
Based on these requirements, the following required information and forms will have the following due dates:
- Group Master Application
- Application for Enrollment/Change or Enrollment Spreadsheet
- Waiver Form
- Affidavit of Qualifying Domestic Partnership (if applicable)
- Affidavit of Qualifying Incapacitated Dependent Eligibility (if applicable)
For groups with Feb. 1, 2011, effective dates, enrollment information will be due on the following dates:
Jan. 20, 2011: Paper Enrollment Applications are due by 5 p.m. (Pacific time).
Jan. 25, 2011: Online or Spreadsheet Enrollment Information are due by 5 p.m. (Pacific time).
To be consistent with these deadlines, Agent Center will close for quoting at 5 p.m. (Pacific time) on Jan. 20, 2011, for quotes for coverage effective Feb. 1, 2011.
Please refer to our Micro Group reference guide for more information about the required documentation for groups of 1 to 4.
Keep in mind that incomplete or inaccurate information will delay the effective date of group coverage.
