02/09/2011   We're enhancing care management services for rare and complex conditions

One in 10 Americans suffer from a rare and complex condition, creating a significant impact on health care costs. We've long addressed complex diseases in our case management program for those members.

In March 2011, we'll introduce the Asuris Rare Disease Condition Management Program, an intensified care management program offered in partnership with Accordant® Health Services. This program enhances our ability to address the complex challenges that patients with these conditions face. It's designed to meet unique health care needs of members with complex, chronic conditions and is available at no additional cost to our members. Specially trained nurses are available 24 hours a day to answer questions, support the physician's care plan, help identify potential concerns, locate resources and information, coordinate care, and monitor changes in the participant's health.

Below is a list of complex and rare conditions that the Asuris Rare Disease Condition Management Program focuses on, along with their estimated prevalence and costs. (Figures are provided by Accordant and are estimates for a health plan with enrollment of 1.5 million members.)

Eligible members will receive:

• Individualized treatment plans
• Personalized counseling and support
• Educational materials, including online resources and tools

The Asuris Rare Disease Condition Management Program is included with fully insured plans, Asuris TruAdvantage^TM, and Individual products. It will also be available as a buy-up option for self-funded groups in 2012.
