Reminder: December submission deadline approaching for new groups size 5 to 99
All required forms must be complete and submitted to us by the published deadlines without exception. Incomplete or inaccurate information will delay the effective date of group coverage.
The following information and forms are required:
- Group Master Application
- Application for Enrollment/Change or Enrollment Spreadsheet
- Waiver Form
- Affidavit of Qualifying Domestic Partnership (if applicable)
- Affidavit of Qualifying Incapacitated Dependent Eligibility (if applicable)
For groups with Jan. 1, 2012, effective dates, enrollment information will be due on the following dates:
- Dec. 20, 2011: Paper Enrollment Applications are due by 5 p.m. (Pacific time).
- Dec. 27, 2011: Online or Spreadsheet Enrollment Information is due by 5 p.m. (Pacific time).
To be consistent with these deadlines, Agent Center will close for quoting at 5 p.m. (Pacific time) on Dec. 20, 2011, for quotes for coverage effective Jan. 1, 2012.
