Individual Standard Health Questionnaire (SHQ) requirement
If a Standard Health Questionnaire (SHQ) is needed, Asuris requires that the full Washington State Standard Health Questionnaire (SHQ) be completed and submitted in its entirety with the application.
If an applicant marks an exception on Page 2 of the SHQ, they still need to submit the entire signed document with their application. We cannot accept just the exception and signature pages of the SHQ. If the applicant marks the exception on the Individual application then no full SHQ is required, they need only submit their exemption proof with the application.
Also, if a change is required, the applicant must resubmit the entire SHQ, not just the changed page. This entire SHQ is required because the applicant's attestation to the accuracy of the information needs to apply to the entire document.
