06/04/2010   Revised Individual Applications now available

Revisions have been made to the Individual Application. The new revision date is 6/10. The new version is available for download on our agent website. Please replace your supply of the old 1/10 application with the new 6/10 version*.

Changes included in this revision are:
  • A new exception was added to the Washington Standard Health Questionnaire:
    • "Are you applying for individual insurance 90 days before or after your employer discontinues your group's insurance due to business closure and you had at least 24 months of continuous group insurance coverage immediately prior to your insurance being discontinued and the effective date of the individual insurance you are applying for is on or within 90 days after the date your group insurance is discontinued?"
  • Applicants are no longer asked to explain why a dependent's name is different from the primary applicant's name.
  • Non-smoker certification language has changed from disaffirming to affirming.
  • Separate non-smoker certification signature was removed—the primary applicant and spouse must still affirm.
  • Deductible and out-of-pocket fields were added under the Other Coverage Information section.
  • There's a new member card level choice available now—all family member names can go on one card or each person can get their own card.
