New member cards going to MotivateSM, EmbarkSM, and VantageSM members
Beginning July 7, MotivateSM, EmbarkSM and VantageSM members will receive new member cards. The new cards will include the following enhancements:
- New design for easy reading
- Provider network icon to show expanded nationwide Asuris Preferred Provider Network
- Number of up-front office visits covered per year (Embark only)
- Office-visit copay (Embark only)
- Vision icon if vision has been purchased
- Health plan name
The cards will be mailed to current members starting July 7 and the mailing will include an insert describing the enhancements. The insert will also include a message letting the member know their benefits have not changed.
