Two Individual enrollment forms no longer required
We are continually working to improve and simplify the application process for enrolling in Individual plans. Most recently this work includes the elimination of the need for the following two forms:
- The Non-Smoker certification document is no longer needed, as members can request the Non-Smoker rate via a telephone call to customer service.
- The Individual Plan Change Form is no longer needed, as a member can use the Individual Application Form to change plans by selecting "Changing Coverage Type" in Section 1.
