Jump Start Sales Program to begin new program period effective July 1
We’re excited to announce that we will begin a new Jump Start Sales Program for new
Individual, Group and Association business with effective dates of July 1, 2007, through
Dec. 1, 2007.

- A contract is defined as a subscriber.
- Once qualification is met, payment is issued retroactive to the first contract

and/or fourth quarter 2007.

A contract is defined as a subscriber/employee and is determined based on enrollment data recorded in our system by the fifth of the following month.

Payment will not be adjusted for enrollment received after the end of the month from the effective date.

Group and Association business qualify separately.

Payment is issued once for each qualifying new group enrolled during the program period.

Payment is issued one month after the qualification threshold is met and is retroactive to the first group qualified during the program.

Payment is limited to $75,000 total during the program period, but capped separately for group business, and for each Association.
Rules
-
Medicare business is excluded from this program and does not count toward qualification.

The terms “broker” and “agent” refer to a company employing one or more producers who are appointed to sell Asuris Northwest Health products.

We may, at any time, amend these rules.
