Generic Ambien® now on pharmacy shelves
The generic medication zolpidem tartrate (brand-name Ambien) recently became available at local pharmacies. Only the regular-release version of zolpidem is available as a generic. Ambien CR is available only as a brand-name (tier three).
For our commercial, Medicare Part D and self-insured clients who have selected our standard RegenceRx® prior authorization, zolpidem will be covered at the generic (tier one) copay. The quantity limit for Ambien (14 tablets) also applies to zolpidem.
Effective immediately, Ambien has moved to non-preferred/non-formulary status and is available at the tier-three copay with the original quantity limit in place.
