Asuris Disease Management programs rolled out to members
In our April issue, we announced that we were expanding our Disease Management programs as an option to all fully insured members and self-funded groups.
We’re now excited to announce that we have begun a large campaign to invite newly identified members into our Disease Management programs, which are designed to provide support to people with the following conditions:
Members should have begun receiving this information as early as June 25.
This mailing included members we identified as people who could benefit from the Asuris Disease Management support programs. Members identified as high-risk are receiving letters signed by the nurse who will follow-up with a phone call.
Please note: Members identified with anxiety and depression were not included in this mailing. They will receive welcome letters as part of a smaller-scale campaign.
