12/18/2006 Updated Employee Enrollment & Change Form is available
The 12/06 version of the Employee Enrollment & Change Form is now available for your use. Please begin using this new version by Dec. 31, 2006.
Also, we no longer require the group administrator to sign the form. However, please note that the following fields in the Employer Section must be completed: “Group Name,” “Intended Effective Date,” “Date of Hire” and “Hours Worked Per Week.”
